In order to make the insurance entry for the internships to be done within the scope of the course or for the internships where insurance entry is required, the internship application documents must be submitted to the Psychology Department Internship Commission at least 15 days before the start date of the internship. Insurance entry is not required for students who are currently insured employees-insured interns or who are accepted by the other institution to perform their internship voluntarily.
The original copies of the internship application documents must be filled in by the student and submitted to the Psychology Department Internship Commission after the relevant parts are signed by the other party. The Career Center (kariyer@fsm.edu.tr) should be contacted for the SSI entry document and insurance entry status. Documents that are not submitted as originals are not processed.

Before you start your internship:
- Internship application form
- Internship acceptance form (Completely filled in 3 original copies will be prepared and signed)
- Work Accident and Occupational Diseases Insurance Form
- Photocopy of identity card
- Residence
- Criminal record (Prosecutor's Office Certificate)
- Health Provision and Activation System (SPAS) Certificate of Eligibility (to be obtained via E-Government)
- 3 photographs (to be pasted on the internship acceptance forms).
To be filled during the internship period:
- Student internship portfolio
To be delivered to the instructor at the end of the internship:
- Student internship portfolio (It will be filled out by the student and approved by the authorized person of the institution where the internship is done, and will be submitted to the department with the evaluation form in the same transparent file in the closed envelope.)
- Internship evaluation form (It will be in a sealed envelope.)
* If the internship is done within the scope of a course, it is required for grading. It is not required for voluntary internships. It can be archived by the student as an internship file (portfolio).
